Unfortunately the actions must be context sensitive.
What's the best way to tackle this practically?

Vitaliy,
Are the comment actions specific to the context they are detected in? Is so, then you may have issues (beyond parsing) in associating the comments with the appropriate language structures. There are many academic papers which discuss this difficult problem.
On the other hand, if the comment actions are not context sensitive, then you should be able to add the action to the comment rule and still mark the comment as hidden so that it doesn't mess up your other rules.
